MISSION
The projects goal is to provide an IT solution that improves the delivery of "Person Centred Planning", which is an
approach created to assist someone to plan their life and supports and is used as a life-planning tool to enable people
with disabilities to increase their self-determination. It wants to benefit service users and their families, RehabCare and
those who commission RehabCare as a service provider, by providing instant access to outcomes thereby enabling
positive changes in the lives of people.

HE SOLUTION THAT HAS BEEN DEVELOPED
Project members said that the past "Person Centred Plans" were good looking on paper, but at times actions were not
achieved and plans often became static as they were recorded on paper and often filed away until the next review. The
staff that should be looking after the plans often did not have the time to really collaborate and stay on track in relation
to plan implementation. There was a reliance on the staff´s interpretation of how to approach the planning process and
what to document and there were only limited possibilities of the family and circle of support to participate in the
"Person Centred Planning". Therefore, the project team created a web-based person centred planning tool that enables
service users to participate in the development and tracking of their own plans, including "Person Centred Plans" and
"Support Plans". The users can grant online access to their plans to family members and members of their "Circle of
Support" in order that they can participate in the planning process. The tool makes the planning process more
streamlined and outcomes and actions can now be monitored and results can be recorded. The intentions of the team
were to provide an easy to use multi-media capability to support "Person Centred Planning", to improve team
collaboration and performance and to provide technology that ensures that actions are honoured. Furthermore, to
provide real-time updating of information and to enable organizations to use the information in plans to easily report on
progress to external stakeholders and inform internal performance management.
